Parental/Guardian Consent (For Minors)

Applicants under the age of 18 must have permission from a parent or legal guardian to participate in the audition process.
A completed and signed Parental/Guardian Consent Form must be submitted along with the application and audition materials.
By signing the consent form, the parent or guardian:
- Confirms they have read and understood the audition’s Terms and Conditions.
- Grants permission for the minor to participate in all stages of the audition process and, if selected, in the production itself.
- Consents to the use of the minor’s likeness, name, voice, and audition materials for all purposes related to the production, including publicity.
- Acknowledges that the production may require additional consents, waivers, or legal documents as necessary for participation.
The production may request proof of identity and relationship (such as a copy of a birth certificate or legal guardianship document).
